Billing

Consultation fees are payable directly after consultation. Patients on medical aid should claim back fees from their respective funds.

First Consultation

At your first consultation Dr van Zyl’s staff will ask you to complete a new patient questionnaire. This will allow the practice to register you and will save you time when you arrive. Or you can download the Patient Registration Form and email it to us ahead of time (preferred).

We also ask that you bring with you your medical aid details, copies of any medical imaging (X – rays, CT scans, MRI scans etc) and or blood tests that you have done.

We also kindly request that you have an up-to-date referral from your GP or healthcare provider as well as a summary of your regular medication.

It is also advisable to wear appropriate clothing, to allow exposure of the body part in question. For example, if you have a knee problem shorts or very loose pants that can be easily rolled up to expose the knee are better than tight jeans.

Hospital Admission Information

Two days prior to admission you should contact us to confirm your admission time, paperwork, fasting time, and any further questions you may have.
